Tile Flooring – Coefficient of Friction (DCOF)

Our tile flooring submittal got approved without any of us catching that it didn’t meet the DCOF requirement. Our inspector caught this after we installed it, we had to pay $20K for a Tile Micro-Etching Subcontractor to fix it!

Our embed shop drawings were approved with incorrect dimensioning, and the majority of embeds were installed 7’’ off. We had to post-install embeds costing us $$$ and weeks of overtime.

Rebar Column Cage Misalignment

Our rebar shop drawings got approved without us realizing the structural and architectural plans had conflicting column sizes – the rebar column cages were installed in the wrong orientation onsite, thankfully we caught this during pre-pour inspection, and spent overtime hours to fix it before pouring the next day. It was a stressful and costly mistake that could have been avoided
